Buddy start

1)Starter control valve failure

2)BA valve failure

3)Electrical wiring or power failure between the start button and the Start Control Valve

4)In a really bad day, no bleed air on that wing due to being isolated because of crosswing duct failure or "puncture", of course that would require both engines on that wing to be buddy started and probably very specific clearance to operate that way.

I am sure there are many many more esoteric causes, like starter gear in the gearbox stripped out or stranger things.
